2009 Gold Star Chili "Flavor of Cincinnati" Trivia and Instant Win Game Promotion
NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING.
  1. Eligibility: The Gold Star Chili "Flavor of Cincinnati" Trivia and Instant Win Game Promotion (The "Promotion") is open to legal residents of Indiana, Kentucky and Ohio only. Must be 13 years of age or older to enter Trivia Game Drawing. Employees of Gold Star Chili, its affiliates and subsidiaries, the judging organization, and any other individual or company involved in the development, promotion, or execution of the Promotion, including advertising, promotional and service agencies, and the immediate families (spouse, parents, children, siblings, and in-laws) and individuals living in the same households of such employees, individuals or companies are not eligible. The Promotion is subject to all applicable federal, state, local laws and regulations. The Promotion void outside Indiana, Kentucky and Ohio, and wherever prohibited by law.
  2. How to Play Game: Between April 13, 2009 and June 14, 2009, obtain an official game piece from Gold Star Chili while supplies last. Peel open the game piece to reveal your instant win prize ("Instant Win Prize"). All game pieces contain an Instant Win Prize of some kind (see list below) that may be redeemed at participating Gold Star Chili restaurants or at www.goldstarchilli.com (depending on Instant Win Prize) through August 31, 2009. Official game pieces can also be used to enter the Trivia Game Drawing (see below). Game pieces will not be available after supplies are exhausted or June 14, 2009, whichever is earlier.
  3. How to Obtain an Official Game Piece:
    1. Purchase a combo meal at participating Gold Star Chili restaurants. Customers of participating Gold Star Chili restaurants will receive one (1) official game piece with the purchase of a combo meal starting on April 13, 2009 and ending June 14, 2009 (or earlier if supplies are depleted).
    2. Non-Purchase Method. To request a free game piece, send a self-addressed envelope to: Gold Star Chili "The Flavor of Cincinnati" Trivia Game, Gold Star Chili, Inc., 650 Lunken Park Drive, Cincinnati, OH 45226, Attn: Trivia Game Piece Request. Requests must be received by June 14, 2009. Sponsor will send one (1) official game piece per each self-addressed envelope. Only one (1) self-addressed envelope may be included with each request/mailing, but multiple individual requests may be sent during the Promotion period.
  4. How to Enter Trivia Game Drawing: Each official game piece includes a trivia question. To enter the Trivia Game Drawing, scratch off the correct answer to the trivia question, complete the official entry form on the back of the game piece and mail it to "Flavor of Cincinnati" Trivia Game, PO Box 15699, Cincinnati, OH 45215-9783. All official game pieces that reveal the correct answer to the trivia question (and only reveal one answer), provide all of the required information on the entry form, and that are received by Sponsor by July 3, 2009 ("Eligible Entries"), will be included in the Trivia Game Drawing. Limit one (1) Eligible Entry per official game piece. Game pieces revealing more than one answer are not eligible. Participants may submit more than one Eligible Entry. Automated methods of entry are prohibited. Sponsor reserves the right to use each entrant's email address for marketing purposes, but all such entrants may opt-out of receiving such communications by following the opt-out procedures set forth in any such marketing communication. In addition, by submitting an entry, all entrants will be automatically enrolled in the Gold Star Chili eClub (http://www.goldstarchili.com/eclub) unless the appropriate opt-out selection is made on the entry form.
  5. Instant Win Prize Claims: Instant Win Prizes may be redeemed at participating Gold Star Chili restaurants through August 31, 2009. Limit one (1) prize redemption per customer per restaurant visit. No substitutions or cash redemptions permitted. Discounted or free items are subject to availability. Instant Win Prizes may not be combined with other promotional offers. Only original game pieces and Instant Win Prizes will be accepted and neither may be re-sold.
  6. Trivia Game Random Drawings: On or about July 6, 2009, random drawings will be conducted from among all Eligible Entries. One (1) Grand Prize and one hundred seventeen (117) Secondary Prizes will be awarded based on the drawings (total AVR for all prizes: $10,262). Sponsor's decisions are final on all matters relating to the Promotion. Odds of winning a Trivia Game Drawing prize will depend on the total number of Eligible Entries. Limit one (1) Trivia Game Drawing prize per person/household.

    No substitution, cash redemption or transfer of any prize by winner permitted. Except where specifically stated, dates and times of all tickets will be determined in Sponsor's sole discretion. All prizes are subject to the terms and conditions of the tickets, venue and issuer. All federal, state and local taxes are the sole responsibility of winner. Sponsor reserves the right to substitute any prize with one of equal or greater value.
  8. Trivia Game Prize Winner Notification: All potential Trivia Game Prize winners will be notified by telephone (no messages will be left) or email on or about July 6, 2009. All Trivia Game Prize awards are subject to in-person verification. Each potential winner will be required to come to Gold Star Chili, Inc. located at 650 Lunken Park Drive, Cincinnati, OH 45226 and provide a valid driver's license or other government photo identification. To claim a Trivia Game Prize, potential winners must sign an Affidavit of Eligibility and Liability/Publicity Release (except where prohibited). Failure to comply with these requirements within 14 days of attempted notification or by Monday, July 20, 2009, will result in forfeiture of the Trivia Game Prize in its entirety and the selection of an alternate Trivia Game Prize winner.
  9. Additional Terms: By accepting a Trivia Game Prize, winners consent to the use of their names, likenesses, voices and/or biographical information for advertising and promotional purposes without additional compensation, except where prohibited by law. By entering any portion of the Promotion, participants agree to be bound by these Official Rules and agree to release and hold Sponsor, its affiliates, subsidiaries, and each company's respective officers, directors, employees, independent contractors, representatives, and agents, including the judging organization, including advertising, promotional and service agencies and any other individual or company involved in the development, promotion, or execution of the Promotion (collectively "Releasees"), harmless from and against any claim or cause of action arising out of participation in the Promotion or the receipt, redemption or use of any prize. By participating in this promotion, you agree to be bound by the decisions of the Sponsor or its judging organization, which are final and binding on all matters. Sponsor reserves the right to modify the prize award procedures at its discretion. Sponsor may prohibit an entrant from participating in the Promotion or winning any prize if, in its sole discretion, it determines that the entrant is attempting to undermine the legitimate operation of the Promotion by cheating, hacking, deception, or other unfair practices or intending to annoy, abuse, or threaten any other entrants or Sponsor's representatives.
  10. Limitation of Liability: Releasees are not responsible for printing, typographical, human or other errors of any kind in any promotion-related materials; for stolen, lost, late, misdirected, damaged, incomplete, illegible, or postage-due mail or entries; or for transactions that are lost, misdirected, or fail to enter into the processing system; or are processed, reported, or transmitted late or incorrectly; or are lost for any reason, including computer, telephonic, paper-transfer, or human error, or electronic, computer, or telephonic malfunction, damage, error, omission, interruption, defect, delay in operation or transmission, or any combination thereof. Sponsor's liability for defective game pieces is limited to replacement with another game piece, while supplies last. Any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Sweepstakes, but in no event attorney's fees. Sponsor will not award more than the number of Instant Win Prizes stated below for any reason. If, for any reason, this Promotion (or any portion thereof) cannot be performed as planned, or infection by computer viruses, bugs, or tampering, unauthorized intervention, fraud, actions of entrants, technical failures, or any other causes corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Promotion, Sponsor reserves the right at its sole discretion to disqualify any entry or entrant and/or to cancel, terminate, modify, or suspend the Promotion and award prizes from among all eligible non-suspect entries validly received prior to the action requiring such modification. By entering, all entrants agree that this Promotion is governed by laws of the state of Ohio for the resolution of all claims and disputes.

  12. Winners List: For the names of Trivia Game Prize winners, mail a self-addressed, stamped envelope to: Gold Star Chili, Inc., 650 Lunken Park Drive, Cincinnati, OH 45226, Attn. Trivia Game Winner List. All such requests must be received by August 7, 2009. The winners list will be sent when all drawings have been conducted, all winners have been verified, and all prizes have been awarded.
